Volunteer Application

In consideration of being permitted to participate in activities conducted under the auspices of IN OUR BACKYARD (“IOB”) to counteract the trafficking of humans (the “Covered Activities”), the undersigned hereby for itself and its members, managers, officers, directors, employees, agents, representatives and others acting by or through them (collectively, “Releasors”), waives and releases all claims and rights that the undersigned may have now or in the future against IOB and its members, managers, officers, directors, employees, agents, representatives and others acting by or through them, arising out of or in connection with (a) any accident or injury (including death) to the undersigned or any other person or damage to any property arising from or related to Covered Activities, or (b) any act, error, omission, fault, negligence or strict liability of any Releasee in connection with Covered Activities, and (iii) agrees not to bring any lawsuit or file any charge or claim against any Releasee in any court or before any government agency related to Covered Activities.

I hereby consent and authorize an employee or agent of IN OUR BACKYARD to take photographs or motion pictures of me; or to produce videotapes, audiotapes, closed-circuit television programs, webcasts, or other types of media productions that capture my name, voice, and/or image (any of the foregoing types of media are called the “Materials” in this Consent and Release form). I hereby waive and release any and all copyrights and other proprietary rights that I may have in the Materials. I authorize IN OUR BACKYARD to copyright the Materials, and I authorize IN OUR BACKYARD to use, reuse, copy, publish, display, exhibit, reproduce, license to third party, and distribute the Materials in any educational or promotional materials or other forms of media, which may include, but are not limited to non-profit publications, videos, articles, websites or publications, electronic or otherwise, without notifying me. I also agree that IN OUR BACKYARD may identify me by name, title, occupation, and such other identifying information. I agree that I am participating on a voluntary basis and I will not receive any payment from IN OUR BACKYARD for signing this release or as a result of any publication of the Materials. I represent that I am at least 18 years of age, or if not, the signature below is the signature of my parent or legal guardian.

In consideration of being permitted to participate in activities conducted under the auspices of IN OUR BACKYARD (“IOB”) to counteract the trafficking of humans (the “Covered Activities”), the undersigned hereby (i) agrees to obtain from the proprietor of any business or location in which said stickers are placed a signed consent and waiver in IOB’s standard form, and (ii) for itself and its members, managers, officers, directors, employees, agents, representatives and others acting by or through them (collectively, “Releasors”),waives and releases all claims and rights that the undersigned may have now or in the future against IOB and its members, managers, officers, directors, employees, agents, representatives and others acting by or through them, arising out of or in connection with (a) any accident or injury (including death) to the undersigned or any other person or damage to any property arising from or related to Covered Activities, or (b) any act, error, omission, fault, negligence or strict liability of any Releasee in connection with Covered Activities, and (iii) agrees not to bring any lawsuit or file any charge or claim against any Releasee in any court or before any government agency related to Covered Activities. I have read and I understand the above Agreement and release for IN OUR BACKYARD. I agree to abide by the rules described above and understand that I may be removed as a participant if I violate any of these rules.

The following policy provides guidelines and establishes procedures for IN OUR BACKYARD (IOB)
employees, board members, and volunteers working with victims/survivors of human trafficking.
As an employee, board member, or volunteer of IN OUR BACKYARD, I agree with the following
statements and will adhere to them at all times, both while working with IN OUR BACKYARD and
following termination of my relationship with IOB.
Confidentiality requires protecting another person’s right to privacy and is vital for working with
victims/survivors of human trafficking.
During my employment, volunteer experience or other relationship with IN OUR BACKYARD, I will have
access to personal information about victims and survivors. I agree that I am responsible for the security
of all such information that is disclosed to me and that I need to maintain confidentiality to protect the
personal information from unlawful or improper release. If I have questions about what I am able to tell
others, I will ask my supervisor prior to providing any information.
I agree that personal information about colleagues or others associated with IOB, names and other
identifying information about victims/survivors, and business records, files, reports, and other documents
of IOB or containing information regarding IOB, IOB’s personnel or others associated with IOB, are to be
used only for their intended purposes as designated by IOB. (Information discussed in open court is not
considered confidential.)
I hereby agree to regard as confidential, and to maintain confidentiality of, all information that I receive
and/or that is discussed or otherwise disclosed in my presence during my performance as an employee,
intern, volunteer or board member.
I agree to maintain confidentiality with staff, volunteers, and clients. I will not share confidential
information with family, friends, acquaintances, or media. I further agree not to remove from IOB’s
facilities or make copies of any records, reports or other documents owned or prepared by or for IOB
without the prior approval of my supervisor. I understand that unauthorized release of confidential
information could result in termination or other disciplinary action.
I have read the foregoing confidentiality agreement, and I agree to adhere to the policies and procedures
stated therein.
